Community Scholarship Program Application
Submission Instructions:
Submit complete application packet to by March 4, 2011. Applications should be either typed or printed in black ink. Applications in pencil will not be considered.
Application – Required Items:
1. Transmittal Form
2. Completed scholarship application form
3. Applicant appraisal form from teacher or counselor
4. Second applicant appraisal form
5. High school transcript
6. Essay (maximum two typewritten pages)
7. Place items 1-6 in sealed 9.5 X 11 in envelope - DO NOT FOLD
